NEW! Action Taken at the September ACGME Board of Directors Meeting: New Requirements Approved

At its September meeting, the Board of Directors approved the new Program Requirements for Combined Programs, effective September 30, 2024. The Requirements and FAQs are available under the “Combined Program Requirements Documents” heading on the new Combined Programs page of the ACGME website.

Designated institutional officials (DIOs) and program directors of combined programs currently listed in the Accreditation Data System (ADS) will receive an email describing the opt-in process.

The Institutional Review Committee (IRC) and ACGME have developed a two-part webinar series on the major revision of the Institutional Requirements. This content is available in Learn at ACGME. No login or registration is necessary to access the videos or download the corresponding slides.

The proposed Institutional Requirements represent the continuing evolution of the Sponsoring Institution, its programs, and its relationship to clinical learning environments for resident and fellow education. To provide enhanced opportunities for feedback and engagement among GME stakeholders, the IRC plans to offer two 45-day review and comment periods.

NEW! Non-Standard Training (NST) Programs Office Hours

Director, Sponsoring Institutions Projects Katharina Wang and Vice President, Sponsoring Institutions Paul Foster Johnson will host office hours to answer questions about recognition of Sponsoring Institutions with NST programs for physicians with J-1 visas.

No registration is required. Office hours will take place October 30 from 12:00-1:00 p.m. Central.

NEW! NST Programs Site Visit Date Re-Assignments

The IRC has approved a plan to reassign approximate site visit dates for Initial Recognition of Sponsoring Institutions with NST programs beginining in October 2025. The reassignment of dates will provide additional time for Sponsoring Institutions to develop NST programs while the IRC prepares further guidance in advance of implementing the site visit process. Each Sponsoring Institution with an Initial Recognition status will soon receive notification of its reassigned site visit date in the Accreditation Data System (ADS).

NEW! Participate in the Obstetrics and Gynecology Flexible Procedural Experience Pilot

The Review Committee for Obstetrics and Gynecology announces a vaginal hysterectomy (VH)/operative vaginal delivery (OVD) pilot in effect immediately for 2025 graduates. The volume of these procedures has declined nationally although they remain very important in the care of the patients the specialty serves.

The goal of the pilot is to consolidate these procedures into the hands of those who will most likely continue to do them in practice. Total program minimums will not change. A program with four residents per year will be required to show 60 VH/OVDs annually, however based on the new pilot guidelines, the distribution of cases per resident may vary. Programs not meeting these total program minimums will be cited. All residents are expected to have equitable educational experiences until The Match®.

Additionally, resident experience cannot be “zero” in VH or OVD, regardless of The Match®. For those residents who match into maternal-fetal medicine, gynecologic oncology, reproductive endocrinology and infertility, and urogynecology and reconstructive pelvic surgery programs, fewer than 15 procedures will be allowable as follows:

  • Residents matched into gynecologic oncology, reproductive endocrinology and infertility, and urogynecology and reconstructive pelvic surgery programs can have fewer than 15 OVDs without citation.
  • Residents matched into maternal fetal medicine, gynecologic oncology, and reproductive endocrinology and infertility programs can have fewer than 15 VHs without citation.

All programs may participate in the pilot program, and do not need to enroll. Programs are asked to self-identify participation in the pilot and share specific resident education experiences related to the pilot in the “Major Changes” section of the ADS Annual Update.

DEADLINE EXTENDED! ACGME Seeks Appeals Panel Members

The ACGME is seeking physicians to serve on standing Appeals Panels for the following specialties:

  • Dermatology
  • Neurological Surgery
  • NEW! Orthopaedic Surgery
  • Preventive Medicine
  • Vascular Surgery

Applicant Criteria

  • Must be certified by the relevant American Board of Medical Specialties member board or American Osteopathic Association certifying board.
  • Must be able to devote sufficient time to the Appeals Panel work.
  • Should have at least three years' experience as a program director, department chair, associate/assistant program director, and/or core faculty member.
    • Applicants not currently serving in one of these roles may be considered if they held one of these positions within the last three years.
  • Applicants’ programs must have a status of Continued Accreditation.

Review Committee for Otolaryngology – Head and Neck Surgery Office Hours

Program leaders can meet with the ACGME Review Committee executive director and chair during the 2024 combined meeting of the Society of University Otolaryngologists and Head & Neck Surgeons, Association of Academic Departments of Otolaryngology and Head & Neck Surgery, and Otolaryngology Program Directors Organization.

Appointments will be held on a first-come, first-served basis at the Hyatt Regency Phoenix on November 22, 2024.

Flexibility in Surgical Training (FIT) Requests

The Review Committee for Surgery has refined its procedure for review and approval of FIT requests. Effective immediately, programs should no longer submit a separate request for acknowledgement and approval of FIT to the American Board of Surgery (ABS). The Review Committee will still send Letters of Notification for approved FIT requests. Programs must submit FIT approval letters for each applicable rotation to the ABS as necessary for individual resident certification. The Review Committee will not provide separate approval letters to the ABS.

ACGME Seeks Public Members

The following Committees seek their next public member for a six-year term beginning July 1, 2025. Public members share the perspective of the public, foster public accountability, enrich the work of the Committee through outside expertise, and create transparency. While nominees cannot be physicians, other health care practitioners are permitted if they are not employed by an organization with ACGME-accredited or -recognized residency or fellowship programs.

A Targeted Clinical Reasoning Remediation Program for Residents and Fellows in Need

Despite clinical reasoning being commonly cited as a deficiency in residents needing remediation, there is little published literature to guide individualized remediation of learners who struggle with this skill. This five-year observational study looks at a remediation program that targets specific subtypes of reasoning deficiencies.

On Dobbs: Our First Year of US OB/GYN Residency
“We are scared.” So concludes this Perspectives article written by obstetrics and gynecology residents in Missouri (who have requested anonymity). Recounting their experience with delayed and denied care for such medical emergencies as postpartum hemorrhage and ectopic pregnancy, the authors also discuss the effect of the US Supreme Court decision in Dobbs v. Jackson Women's Health Organization on their specialty, as physicians across the country are placed at criminal risk for providing routine care.

Medically Underserved Areas and Populations Interest Group

The ACGME hosts a monthly virtual interest group session for GME community members involved in addressing the needs of MUA/Ps, which are places or communities with groups of people who have unmet health or health care needs.

Apply for the CLE Quadruple Aim Initiative Pilot

The CLER Program is seeking up to 10 teams from clinical learning environments (CLEs) of ACGME-accredited Sponsoring Institutions to participate in the pilot cycle of a new improvement network, the CLE Quadruple Aim Initiative. This initiative aims to enhance GME integration into CLE efforts to optimize patient care and advance the Quadruple Aim of simultaneously improving patient outcomes, enhancing patient experience, improving health care practitioner well-being, and reducing cost of care.

Selected teams will learn from one another by participating in joint learning sessions while engaging in a 15-month clinical quality improvement project of strategic priority to their clinical site. The application deadline is October 18, 2024.
