NEW! Updated Case Log Calculations for Adult Cardiothoracic Anesthesiology
The Review Committee for Anesthesiology has updated the Case Log category calculations for adult cardiothoracic anesthesiology. It was determined that some cases were not properly calculating under the “Cardiac Surgery with CPB” and/or “Cardiac Surgery with or without CPB” categories. The Review Committee advises programs to run end-of-year Case Log reports again prior to conducting any Clinical Competency Committee meetings.
Upcoming changes to the Case Log minimum requirements will not go into effect until the 2025-2026 academic year.

NEW! Upcoming Major Revision of Program Requirements for Micrographic Surgery and Dermatologic Oncology
The Review Committee for Dermatology invites comments related to specific requirements or topics that should be addressed as revisions are developed. In particular, the Review Committee invites comments related to what the subspecialty will look like in the future, and the competencies, resources, and personnel necessary to create physicians who will practice in this subspecialty.
Changes to the ACGME Common Program Requirements are not within the purview of the Review Committee and, therefore, comments related to Common Program Requirements, indicated by bold font in the Requirements document, should not be submitted and will not be considered.

NEW! Orthopaedic Procedure Log Update
A reminder to programs that the procedural minimums for orthopaedic surgery were updated in ADS in mid-January with an effective date of July 1, 2025. As programs are also aware, this is an evolution from the previous case minimums that will continue over the next several years.
Implementation Timeline – Areas for Improvement (AFIs) and Citations:
- July 2025 graduates (current PGY-5 residents) – Review Committee will not cite programs for missed graduating resident minimums.
- July 2026 graduates (current PGY-4 residents) – Review Committee will not cite programs for missed graduating resident minimums but may issue AFIs at its January 2027 Review Committee meeting.
- July 2027 graduates (current PGY-3 residents) – Review Committee will begin issuing citations for missed minimums to programs at the January 2028 Review Committee meeting.
Since January, the Review Committee has made several corrections to CPT codes based on feedback from programs and residents. A real-time list of CPT codes and tracked categories is available in ADS: select the Case Logs menu tab, click on “Download Reports” from the drop-down menu, and select “Tracked Codes” from the left column of the screen from the report list.

Family Medicine Data Collection Tutorial Video and FAQs Available
Family medicine specialty data will now be collected using the ACGME Case Log System, starting with the 2024-2025 academic year, effective May 27, 2025.
A tutorial video and FAQs are now available to help programs with this change in data collection. Both are accessible from the Documents and Resources page of the Family Medicine section of the website.
Upcoming Major Revision of the Osteopathic Recognition Requirements
The Osteopathic Recognition Committee will soon begin work on the 10-year major revision of the Osteopathic Recognition Requirements. To inform the next set of Recognition Requirements, a series of activities designed to gather relevant information is planned.
As this work begins, the ACGME seeks current Directors of Osteopathic Education (DOEs) and/or current program directors from programs with Osteopathic Recognition who are interested in participating in a one-on-one, one-hour interview to provide their perspective on the future of Osteopathic Recognition.
The ACGME is also seeking current DOEs and/or program directors from programs with Osteopathic Recognition who are interested in participating in the Osteopathic Recognition Stakeholder Summit, scheduled to take place in person in Chicago in 2026 (date TBA).
NEW! ADS and Case Log System Downtime due to Academic Year Rollover
ADS and Case Logs will be unavailable beginning Friday, June 27 from 11:59 p.m. Central until approximately 4:00 p.m. Central on Saturday, June 28 to convert to the 2025-2026 Academic Year.
After 4:00 p.m. June 28, the ACGME's data systems will reflect Academic Year 2025-2026, and all residents and fellows will have an unconfirmed status.
NEW! ADS Annual Update Schedule
The ADS 2025-2026 Academic Year Annual Updates will be collected between July and September 2025, and programs will receive an initial notification email at the beginning of the reporting timeframe noting all Annual Update requirements and their respective deadlines.
Each program's Annual Update timeframe will appear in ADS under the Program > Important Dates column on the right-hand side by July 1. The process for completing the Annual Update remains unchanged with the introduction of ACGME Cloud.
The 2025-2026 ADS Annual Update Overview, Summary of Changes, FAQs, Schedule by Specialty, and other resources will soon be available in the Annual Update section of the ADS Help Center.
Milestones Reporting Window
The Milestones reporting window is open through June 27. Milestones assessments can be submitted via ADS at any point during the reporting window. FAQs can be found on the ACGME website.

ACGME Seeks Appeals Panel Members
The ACGME is seeking physicians to serve on standing Appeals Panels for the following specialties:
- Diagnostic Radiology
- Institutional Review
- Ophthalmology
- Osteopathic Neuromusculoskeletal Medicine
- Radiation Oncology
Apply by June 30, 2025.
Applicant Criteria
- Must be certified by the relevant American Board of Medical Specialties member board or American Osteopathic Association certifying board.
- Must be able to devote sufficient time to the Appeals Panel work.
- Should have at least three years’ experience as a program director, department chair, associate/assistant program director, and/or core faculty member.
- Applicants not currently serving in one of these roles may be considered if they held one of these positions within the last three years.
- Applicants’ programs must have a status of Continued Accreditation.

Coordinator Advisory Group Nominations
The ACGME is accepting nominations for the Coordinator Advisory Group, which serves as a consultative body to the ACGME administration concerning coordinator, GME, learning environment, and accreditation matters. Members will serve three-year terms, from July 1, 2026 to June 30, 2029.

Enhancing Well-Being: Managing Toxic Perfectionism and Other Problematic Mindsets Video Workshop for Coordinators
This 90-minute workshop is designed to equip coordinators with skills and strategies to enhance their satisfaction with work, life, and self. The workshop is divided into three segments, each focusing on different aspects of personal well-being.
Videos can be viewed individually; no login is required.
Developing Faculty Competencies in Assessment
Participants learn how to build effective assessment programs for their residency or fellowship, with topics including assessment methods, feedback, direct observation, Clinical Competency Committees, and more.
The course is being held in a split format July 23-25 and October 15-17 (registrants attend on all session dates). Registration closes June 27.

Leadership Skills Training Program for Chief Residents

This three-day program in Chicago provides a transitional experience into the role of chief resident. It introduces concepts to enhance individual effectiveness in a leadership role and is open to chief residents from all medical and surgical specialties.

This Perspectives article presents a five-step, systematic approach to addressing the cognitive biases that can result from an overreliance on mental shortcuts that educators might use to deal with cognitive overload. The SKAIR Mnemonic encourages faculty to Slow Down, Know Your Bias, Consider the Alternative, Get More Information, and Reflect, providing a practical tool for GME programs to ensure fair, accurate, and objective resident assessments.

Medical errors resulting in patient harm are often the result of miscommunication during transitions of care, also referred to as “hand-offs.” This ACGME News and Views article provides resources for implementing training, observation, and feedback that can significantly improve patient safety. Based on literature that studies both temporary (shift-to-shift) and permanent (end-of-rotation) hand-offs, and incorporating tools such as "Relief in Place" (colloquially known as “left seat, right seat”) from military models, these tools can ensure that residents are competent in these critical communication skills.
